The Trustees of Catholic Aged Care Sydney (TCACS) is the approved provider of resi- dential aged care at John Paul Village (Facility). TCACS has delegated authority to the Parish Priest of St. John Bosco, Engadine in relation to the day

 

-to-day conduct of the Facility.

 

TCACS, the Parish and the Facility (referred to as “we”, “our” and/or “us” throughout this docu- ment) are committed to protecting the privacy  of individuals who provide personal information to us, including those who provide personal in- formation via TCACS, the Parish and/or the Facility’s website; johnpaulvillage.com.au (Site).

 

We are committed to complying with the Priva- cy Act 1988 (Cth) and all applicable principles governing the handling of personal information. This Privacy Policy is designed to tell you about our practices in relation to the collection, use, disclosure and storage of personal information in relation to the Facility.

What is Personal Information?

 

Personal information is any information about you that identifies you or from which your iden- tity reasonably can be determined.

We collect and hold personal information that includes (but is not limited to):

Why do we collect personal information?

We collect and hold personal information for various reasons (which we generally notify to you at the time of collection), including the following primary purposes

  • to enable you to apply to receive and/or receive health and aged care services;
  • In relation to any enquiries about our Residential aged care facilities; and
  • To enable you to contract or supply goods and/or services to us.
How do we use your personal information?

 

Our usual practice is to collect personal information directly from you. In addition to collecting personal information from you, we sometimes collect personal in- formation from a third party if it is unreasonable or impracticable to collect the information from you directly.

 

Your information is usually stored securely as physical documents and/or securely on our computers and servers.

Do we collect information via passive collection?

We may use cookies to assign your computer a User ID so that we can identify your computer on our server. You may configure your web browser so that you are notified in ad- vance so your internet browser will disable cookies. In some instances cookies may collect and store personal information about you. Any personal information collected via the use of cookies will only be used by us in accordance with this Privacy Policy.

Your Consent

In most cases, we require you to consent specifically to any collection, use or disclosure of your personal information by us. Your consent may be explicit, such as in writing or verbally, or may be implied by conduct.

How will we use personal information you provide

We use and discloses the personal information you provide for the specific purpose or purposes for which you have given consent and for secondary purposes for which you reasonably expect us to use or disclose your personal information. We do not disclose your personal information to overseas recipients.

(ii) your personal information being  used to improve the Site; and

If we collect information about you and you would reasonably expect us to use or disclose the information for the purpose of marketing, we will ensure that an easy-to-use, unsubscribe or opt-out procedure is available to you in all our marketing communications.

In addition, if you provide personal information to us via the Site, you agree that we may combine such information with other actively collected information unless we specify otherwise at the point of collection.

Do we disclose personal information to anyone.

We may disclose your personal information in a number of circumstances, e.g :

(i) where you have consented to us doing so; or

(ii) where required or authorised by law; or

(iii) to consultants, advisers or other third parties where that party has been contracted to provide administrative or other services to us (and on the basis that that third party agrees to abide by this Privacy Policy).

Management & Security of personal information.

We take all reasonable precautions to safe- guard your personal information from loss, misuse, interference, unauthorised access, modification or unlawful disclosure.

However, you should keep in mind that the transmission of information over the internet is not completely secure or error-free. In particular, e-mail sent to or from the Site may not be secure, and you should therefore take special care in deciding what information you send to us via e-mail.

Access to personal information.

You may request access to personal information that we hold about you or you can ask us to correct, amend or delete any personal information being held by us about you (see our contact details).

There are circumstances where, by law, we may not give you access to the personal information we hold about you. For example, we cannot give you access if it would unreasonably affect someone else’s privacy or if giving you access poses a serious threat to someone’s life, health or safety.

We will take reasonable steps to update or correct, as soon as possible, any information in our possession that you have previously submitted that is inaccurate, incomplete, out- of-date, irrelevant or misleading.

If you have a privacy complaint, please contact us. Your complaint will be reviewed and investigated internally and we will endeavour to resolve your issue efficiently.

Changes to this privacy Policy.

If this Privacy Policy changes, the revised policy will be posted on the Site. Please check back periodically, and especially be- fore you provide any personal information. Your continued use of the Site following any amendments to it will confirm your acceptance of such amendments.

Other Policy.

This Privacy Policy is our general policy in relation to how we deal with personal information. Our self-care residents are also required to comply with the privacy policy that relate to residents of the Facility, a copy of which is attached to each retirement village contract and can be obtained from our office. The provisions of that policy prevail to the extent of any inconsistency between policies.
